Quick answer: how do you calculate Tesamorelin units?

To calculate tesamorelin units, divide the milligrams in the vial by the millilitres of liquid to get the concentration, divide your prescribed dose in milligrams by that concentration to get the volume, then multiply the volume in mL by 100 for units on a U-100 syringe. Tesamorelin doses are usually written in milligrams, so convert first: 600 mcg is 0.6 mg. Example: a 5 mg vial with 2 mL of bacteriostatic water is 2.5 mg/mL, so if your prescription says 0.6 mg, you draw 0.24 mL, which is 24 units.

Pre-mixed tesamorelin: reading the pharmacy label

Tesamorelin calculators online assume a freeze-dried vial, usually 2 mg, 5 mg or 10 mg, that you mix with bacteriostatic water. A compounding pharmacy prescription usually arrives as a liquid, and the label states the concentration in mg/mL instead of a powder weight. Common label strengths are 1 mg/mL and 2 mg/mL.

With a pre-mixed vial there is no water to add: divide your prescribed dose in mg by the label concentration to get mL, then multiply by 100 for units. Because tesamorelin doses are in the milligram range, draws are larger than for most peptides: 0.6 mg from a 2 mg/mL vial is 30 units, so a 0.5 mL or 1 mL syringe is the usual fit. Switch the calculator above to Pre-mixed vial and type the label strength.

One product to keep separate: the FDA-approved branded tesamorelin (Egrifta SV) ships as a 2 mg vial that is reconstituted with 0.5 mL of the sterile water supplied with it, and its label states the formulations are not substitutable. Compounded tesamorelin is a different product with its own label; use the strength printed on the vial you actually have.

Worked example (pre-mixed vial)
label: 2 mg/mL prescription: 600 mcg (0.6 mg)
0.6 mg ÷ 2 mg/mL = 0.3 mL
0.3 mL × 100 = 30 units

Blend vials

Tesamorelin + Ipamorelin blend calculator

Blend vials hold both peptides in the same water, so every unit you draw carries both in the vial's ratio. Enter the tesamorelin dose on your prescription with each peptide's mg from the vial label, and the calculator shows the units to draw and how much ipamorelin comes with them.

The Tesamorelin / Ipamorelin is prescribed as a pre-mixed Tesamorelin and Ipamorelin blend, already reconstituted by a licensed US 503A pharmacy and labeled with the concentration. No BAC water, no ratios to work out.

Start your Tesamorelin / Ipamorelin visit · from $183/mo

Unlike the CJC-1295 / Ipamorelin blends, tesamorelin / ipamorelin vials are often uneven. An 8 mg blend is commonly 6 mg of tesamorelin and 2 mg of ipamorelin: with 2 mL of water that is 3 mg/mL of tesamorelin and 1 mg/mL of ipamorelin, so every unit carries 30 mcg of tesamorelin and 10 mcg of ipamorelin. A prescription of 0.6 mg tesamorelin is 20 units and brings 200 mcg of ipamorelin with it.

The usual mistake is to divide the 8 mg total by the water and call that the concentration. 4 mg/mL is true for the whole solution but for neither peptide, and using it understates tesamorelin by a quarter. Use each peptide's own mg from the label; if the label only prints a total, ask the pharmacy for the split before you draw.

The ratio is fixed inside one vial. If a prescription calls for tesamorelin and ipamorelin amounts that do not match the vial's ratio, that needs a different blend or two separate vials, which is a question for the prescriber and pharmacy.

When tesamorelin units fall between syringe lines

Tesamorelin draws are large by peptide standards. At 2.5 mg/mL a 0.6 mg dose is 24 units, and at 1 mg/mL it is 60 units, which does not fit a 0.3 mL (30 unit) or 0.5 mL (50 unit) syringe. The calculator flags a draw that exceeds the syringe you selected; switch to a 1 mL syringe rather than splitting a dose across two draws without asking your prescriber.

Decimals such as 17.5 units land between the 2-unit marks on most 1 mL syringes. A 0.5 mL syringe is marked every unit and is easier to read for draws under 50 units. If the result is under 2 units or larger than your syringe holds, ask your pharmacist whether a different concentration fits your dose; do not change the amount of water or the dose on your own.

Common questions

Tesamorelin calculator FAQ

How many units of tesamorelin do I draw?

Units = dose (mg) ÷ concentration (mg/mL) × 100. For a 5 mg vial mixed with 2 mL (2.5 mg/mL), a prescription of 0.6 mg is 0.24 mL, or 24 units on a U-100 syringe. The dose itself comes from your prescription.

How much bacteriostatic water do I add to 2 mg of tesamorelin?

Use the volume your pharmacy or prescriber specifies. The water does not change the amount of tesamorelin, only the concentration: 0.5 mL makes 4 mg/mL (40 mcg per unit), 1 mL makes 2 mg/mL (20 mcg per unit) and 2 mL makes 1 mg/mL (10 mcg per unit). Peptides.build prescriptions arrive pre-mixed, so patients never add water.

How much bacteriostatic water do I add to 5 mg of tesamorelin?

Again, follow the pharmacy's instructions. With 5 mg: 1 mL makes 5 mg/mL (50 mcg per unit), 2 mL makes 2.5 mg/mL (25 mcg per unit) and 5 mL makes 1 mg/mL (10 mcg per unit). More water means more units for the same dose, which makes the draw easier to read but may need a 1 mL syringe.

My tesamorelin vial came pre-mixed. How do I convert my dose to units?

Read the concentration on the pharmacy label, for example 2 mg/mL. Divide your prescribed dose in mg by it to get mL, then multiply by 100: 0.6 mg ÷ 2 mg/mL = 0.3 mL = 30 units. Choose Pre-mixed vial in the calculator to do this automatically.

Is the math the same for Egrifta SV and compounded tesamorelin?

The arithmetic is the same (dose ÷ concentration × 100), but the products are not interchangeable. Egrifta SV is a 2 mg vial reconstituted with 0.5 mL of its own sterile water and drawn as a volume printed in its instructions; compounded tesamorelin comes with its own label strength. Follow the instructions for the product you have, and never swap one product's volumes onto the other.

How do I calculate a tesamorelin and ipamorelin blend?

Divide each peptide's mg by the mL in the vial to get two concentrations. Your tesamorelin dose sets the volume you draw, and that same volume delivers ipamorelin at its own concentration. In a 6 + 2 mg vial with 2 mL, 0.6 mg of tesamorelin is 20 units and carries 200 mcg of ipamorelin.

How should tesamorelin be stored once mixed?

Follow the storage line on the pharmacy label. Reconstituted peptides are typically kept refrigerated, not frozen, and used before the beyond-use date on the label. Do not use a vial that looks cloudy or has particles; contact the pharmacy.

Is tesamorelin FDA-approved?

Tesamorelin is FDA-approved under the brand name Egrifta for one indication only, the reduction of excess abdominal fat in adults with HIV-associated lipodystrophy. Compounded tesamorelin is a different product prepared by a 503A pharmacy for an individual patient, and compounded medications are not FDA-approved. A licensed provider decides whether it is appropriate for you.

Is tesamorelin allowed in competitive sport?

No. The World Anti-Doping Agency Prohibited List names tesamorelin among growth hormone releasing hormone analogues (S2), and ipamorelin among growth hormone secretagogues, both banned at all times. Drug-tested athletes should check with their governing body.

Does this calculator tell me how much tesamorelin to take?

No. It performs arithmetic only and does not recommend a dose or schedule. The dose, timing and vial strength come from your prescription and pharmacy label.
