# What is the Tesamorelin / Ipamorelin blend?

Published October 12, 2026 · Educational, not medical advice

## Short answer

The Tesamorelin / Ipamorelin blend puts two growth hormone secretagogues that act on different receptors in one vial: tesamorelin, a stabilized GHRH analog, and ipamorelin, a five-amino-acid ghrelin-receptor agonist. No trial has tested the combination. The blend is a compounded medication and is not FDA-approved; tesamorelin alone holds a brand-name approval for one HIV-related indication, and ipamorelin is not FDA-approved for any use. Both are prohibited in sport.

## What is in the Tesamorelin / Ipamorelin blend?

The Tesamorelin / Ipamorelin blend combines two synthetic peptides that both prompt the pituitary to release growth hormone, but through different receptors. Tesamorelin is a 44-amino-acid analog of growth-hormone-releasing hormone with a chemical group on its first amino acid that protects it from rapid enzymatic breakdown. Ipamorelin is a pentapeptide, five amino acids long, that acts as a selective agonist at the ghrelin receptor, a separate receptor on the same pituitary cells.

The blend exists because the two routes are distinct. A GHRH analog and a ghrelin-receptor agonist are often described together in the secretagogue literature as acting on complementary signals, which is the rationale compounding pharmacies and prescribers give for putting them in one vial. That rationale is a description of mechanism; it has not been tested in a trial of this combination.

## What has the Tesamorelin / Ipamorelin blend been studied for?

The Tesamorelin / Ipamorelin blend has not itself been studied in any published trial. Its evidence is the sum of two separate literatures of very different depth. Tesamorelin has three randomized entries among the studies cited here, all in HIV-associated abdominal fat accumulation: a 2007 trial of its metabolic effects in patients with HIV, a 2010 pooled analysis of two phase 3 placebo-controlled trials in HIV-infected patients with excess abdominal fat, and a 2014 trial measuring visceral fat and liver fat in the same population.

Ipamorelin, by contrast, has a 1998 preclinical pharmacology paper and one 2014 randomized proof-of-concept trial in postoperative ileus after bowel resection that did not meet its primary endpoint. Nothing in either literature tests the pair together, and nothing tests either compound for general body-composition or performance goals in adults without HIV. Readers should hold both halves of that picture at once: one well-studied compound in a narrow setting, one barely studied compound, and an untested combination.

What the published research covers, by component
| Component | Type of evidence | What the studies examined |
| --- | --- | --- |
| Tesamorelin | Randomized controlled trials and a pooled phase 3 analysis (HIV-associated abdominal fat accumulation) | Metabolic effects, excess abdominal fat, visceral and liver fat in HIV-infected patients |
| Ipamorelin | Preclinical pharmacology | Growth hormone release and receptor selectivity in animal and cell models, 1998 |
| Ipamorelin | Randomized proof-of-concept trial (postoperative ileus) | Bowel function after resection; primary endpoint not met, 2014 |
| The combination | No trials | Not studied together |

## How is the Tesamorelin / Ipamorelin blend prescribed?

The Tesamorelin / Ipamorelin blend is prescribed through Peptides.build as a compounded medication: a licensed US provider reviews an online intake and, where appropriate, writes a prescription that a licensed 503A pharmacy fills by dissolving both peptides in a single vial. The vial ships already reconstituted and the label prints a strength for each peptide, because the two are usually present in unequal amounts.

The blend is one of two ipamorelin pairings in the catalog; the other replaces tesamorelin with CJC-1295. Tesamorelin is also available on its own. Which of those, if any, fits a patient's history and blood work is the provider's decision at intake, and the dose and schedule are set by the provider rather than published on this site.

## Is the Tesamorelin / Ipamorelin blend FDA-approved or allowed in sport?

The Tesamorelin / Ipamorelin blend is a compounded medication and is not FDA-approved. Compounded tesamorelin is not FDA-approved. Tesamorelin itself is FDA-approved under a brand name for one indication only, the reduction of excess abdominal fat in adults with HIV-associated lipodystrophy. Ipamorelin is not FDA-approved for any use. The blend prescribed here is prepared by a licensed 503A pharmacy for a specific patient and is not the branded tesamorelin product; compounded medications are not reviewed or approved by the FDA the way mass-manufactured drugs are.

The Tesamorelin / Ipamorelin blend is prohibited in sport at all times, because both components are. The World Anti-Doping Agency names tesamorelin under S2 among growth hormone releasing factors and ipamorelin under S2 among growth hormone secretagogues. Drug-tested athletes should check with their governing body before a prescription is filled.

## How are Tesamorelin / Ipamorelin doses measured?

Tesamorelin / Ipamorelin prescriptions are usually written for the tesamorelin amount, in milligrams, with the ipamorelin arriving in whatever proportion the vial holds; ipamorelin itself is measured in micrograms. Because the two strengths on the label differ, dividing the vial's total weight by its volume gives a figure that is true for the solution and wrong for both peptides. The free tesamorelin calculator on this site has a blend mode that takes each peptide's label strength, shows the units to draw for the prescribed tesamorelin amount, and shows how much ipamorelin comes with it. It is arithmetic only and does not recommend a dose.

Prescription, not research grade

## Prescribed Tesamorelin / Ipamorelin vs research peptides: what is different?

Prescribed Tesamorelin / Ipamorelin is not a research chemical. It is a prescription medication: a licensed US provider reviews your intake first, and a licensed 503A pharmacy compounds it for you by name. Vials sold online as “research use only” skip both steps. What you pay for here is that review, a pharmacy you can name, and a label that says what is in the vial.

A research seller’s certificate of analysis is a document the seller controls: it chooses the lab and the sample, and a PDF can be reused, edited or re-dated. No regulator checks it.

Prescribed Tesamorelin / Ipamorelin compared with research-use-only peptides
| Point | Prescribed through Peptides.build | Research use only |
| --- | --- | --- |
| Access | Online intake reviewed by a licensed US provider; prescribed only when appropriate. | No prescription and no clinical review of whether it suits you. |
| How it's labeled | A prescription medication compounded for you by name. | Commonly sold labeled “research use only, not for human consumption.” |
| Who prepares it | A licensed 503A US compounding pharmacy under state and federal oversight. | Unregulated suppliers, often with no verifiable chain of custody. |
| Testing | Third-party tested. | Certificates of analysis are self-reported when provided at all. |
| Purity and identity | Prepared to a prescription for a specific patient. | Purity, strength, and even identity are unverified by any independent party. |

This compares two care models, not two products. Compounded medications are not FDA-approved, and we do not claim otherwise. What a prescription adds is a licensed provider deciding whether a protocol is appropriate for you and a licensed pharmacy preparing it. Common questions

## Tesamorelin / Ipamorelin FAQ

### Why combine tesamorelin and ipamorelin?

They act on two different pituitary receptors: tesamorelin on the GHRH receptor and ipamorelin on the ghrelin receptor. Combining them is a mechanism-based rationale for signalling the same axis through two routes; no published trial has tested the pair together.

### Is the Tesamorelin / Ipamorelin blend FDA-approved?

No. The blend is a compounded medication, and compounded medications are not FDA-approved. Tesamorelin alone is FDA-approved under a brand name for one indication only, excess abdominal fat in adults with HIV-associated lipodystrophy, and the blend is not that product. Ipamorelin is not FDA-approved for any use.

### Is the Tesamorelin / Ipamorelin blend banned in sport?

Yes. Both components are prohibited at all times by the World Anti-Doping Agency under S2, tesamorelin as a growth hormone releasing factor and ipamorelin as a growth hormone secretagogue. Drug-tested athletes should check with their governing body.

### Has the combination been tested in a clinical trial?

No. Tesamorelin has randomized trials in HIV-associated abdominal fat accumulation and ipamorelin has one negative trial in postoperative ileus, but no study has tested the two together.

### Can the ratio of tesamorelin to ipamorelin be changed?

Not inside one vial. A blend vial carries both peptides in a fixed ratio, so a different ratio means a different blend or two separate vials. That is a question for the prescriber and pharmacy, not something to adjust at home.
