# What is compounded semaglutide?

Published October 12, 2026 · Educational, not medical advice

## Short answer

Compounded semaglutide is semaglutide, a GLP-1 receptor agonist, prepared by a licensed 503A pharmacy for one patient from a prescription. Semaglutide itself is FDA-approved as the branded drugs Wegovy, Ozempic and Rybelsus, and its large randomized trials were run on those products. The compounded medication is not FDA-approved. It is a once-weekly injection prescribed only after a licensed provider reviews your history.

## What is semaglutide, and what does compounded mean?

Semaglutide is a synthetic version of GLP-1 (glucagon-like peptide-1), a hormone the gut releases after a meal. Natural GLP-1 is broken down within minutes. Semaglutide is modified so it lasts about a week in the body, which is why it is given as a once-weekly injection.

Compounded semaglutide is the same active molecule prepared by a licensed 503A compounding pharmacy for one named patient, from that patient's prescription. It is not the branded product: it carries the compounding pharmacy's label, its own concentration and its own beyond-use date, and it has not been through the FDA's review of a finished drug. That distinction matters more for semaglutide than for most compounds, because a branded, FDA-approved version exists to compare it with.

## Is compounded semaglutide the same as Wegovy or Ozempic?

Compounded semaglutide contains the same active ingredient as Wegovy, Ozempic and Rybelsus, but it is a different product. Wegovy and Ozempic are injectable pens and Rybelsus is a tablet, each manufactured to an FDA-approved specification and approved for specific indications. A compounded vial is prepared by a pharmacy for an individual prescription and is not approved for any indication.

In practice the differences a patient notices are the delivery (a vial and insulin syringe instead of a pre-filled pen), the label (the pharmacy's name, your name and the concentration in mg/mL), and the cost. The difference a regulator cares about is that the compounded product has no FDA approval of its own.

## What has semaglutide been studied for?

Semaglutide has some of the largest randomized trial programs of any medication in its class, all run on the branded products. The STEP 1 trial, published in 2021, compared once-weekly semaglutide with placebo in adults with overweight or obesity who did not have diabetes, alongside lifestyle counseling. The SELECT trial, published in 2023, followed adults with established cardiovascular disease and overweight or obesity, without diabetes, to see whether semaglutide changed the rate of major cardiovascular events.

Semaglutide has also been compared head to head with tirzepatide, in type 2 diabetes in the SURPASS-2 trial and in obesity in the SURMOUNT-5 trial. What none of these trials studied is a compounded product. They establish what semaglutide does as a manufactured drug; they do not test any particular pharmacy's preparation.

## Is compounded semaglutide safe?

Compounded semaglutide carries the same known cautions as the molecule itself, plus the questions that come with any compounded product. The branded products carry a boxed warning about thyroid C-cell tumors seen in rodents, so semaglutide is not prescribed to people with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia syndrome type 2. The labeling also lists pancreatitis, gallbladder problems and stomach side effects such as nausea, which is why the strength is raised gradually over the first weeks rather than started high.

The compounding questions are about the vial: who made it, under what standards, and whether the label is accurate. A licensed 503A pharmacy compounds for a named patient under its state board of pharmacy and sterile compounding standards, and a pharmacist is accountable for the batch. That is a different situation from semaglutide sold online as research use only, which comes with no prescriber, no pharmacy and no screening.

- Ask who prescribed it, by name, and which pharmacy's name is on the label.
- Check that the concentration is printed on the vial in mg/mL.
- Tell your provider about thyroid history, pancreatitis, gallbladder problems and any other diabetes medication before the first prescription.

## How is compounded semaglutide prescribed?

Compounded semaglutide is prescribed through Peptides.build after an online medical intake. A licensed US provider reviews your history, medications and allergies and prescribes it only if it is appropriate; a licensed 503A pharmacy then compounds it and ships it cold-chain, already in solution with the concentration on the label.

The provider sets the starting strength, the pace at which it is raised and the schedule, and adjusts them at check-ins. None of those figures are published on this site, because the right ones depend on the individual. The weekly injection is drawn with an insulin syringe; the number of units comes from the provider's directions and the label concentration.

Prescription, not research grade

## Prescribed Compounded semaglutide vs research peptides: what is different?

Prescribed Compounded semaglutide is not a research chemical. It is a prescription medication: a licensed US provider reviews your intake first, and a licensed 503A pharmacy compounds it for you by name. Vials sold online as “research use only” skip both steps. What you pay for here is that review, a pharmacy you can name, and a label that says what is in the vial.

A research seller’s certificate of analysis is a document the seller controls: it chooses the lab and the sample, and a PDF can be reused, edited or re-dated. No regulator checks it.

Prescribed Compounded semaglutide compared with research-use-only peptides
| Point | Prescribed through Peptides.build | Research use only |
| --- | --- | --- |
| Access | Online intake reviewed by a licensed US provider; prescribed only when appropriate. | No prescription and no clinical review of whether it suits you. |
| How it's labeled | A prescription medication compounded for you by name. | Commonly sold labeled “research use only, not for human consumption.” |
| Who prepares it | A licensed 503A US compounding pharmacy under state and federal oversight. | Unregulated suppliers, often with no verifiable chain of custody. |
| Testing | Third-party tested. | Certificates of analysis are self-reported when provided at all. |
| Purity and identity | Prepared to a prescription for a specific patient. | Purity, strength, and even identity are unverified by any independent party. |

This compares two care models, not two products. Compounded medications are not FDA-approved, and we do not claim otherwise. What a prescription adds is a licensed provider deciding whether a protocol is appropriate for you and a licensed pharmacy preparing it. Common questions

## Compounded semaglutide FAQ

### Is compounded semaglutide FDA-approved?

No. Semaglutide is FDA-approved as the branded drugs Wegovy, Ozempic and Rybelsus. Compounded semaglutide is prepared by a licensed 503A pharmacy for an individual patient and is not FDA-approved.

### Is compounded semaglutide the same as Ozempic?

Same active ingredient, different product. Ozempic is a manufactured pen approved for specific indications; compounded semaglutide is a pharmacy preparation for one patient's prescription, supplied in a vial, and is not FDA-approved.

### Is compounded semaglutide safe?

It carries the molecule's known cautions, including a boxed warning about thyroid C-cell tumors in rodents and warnings about pancreatitis and gallbladder problems, so a provider screens for them first. A licensed 503A pharmacy prepares it under sterile compounding standards, but the compounded product itself has not been reviewed by the FDA.

### How often is semaglutide injected?

Once a week, as a subcutaneous injection. The provider raises the strength gradually over the first weeks and sets the schedule.

### Why does Peptides.build not publish semaglutide doses?

Because the starting strength and how fast it is raised depend on your history and how you respond. The provider sets them and prints the directions on the pharmacy label.
